The North-East club are one of a number of clubs, including Reading and Norwich, to be linked with the player in recent weeks.
It now appears the Mackems have won the race for the Swansea striker’s signature after agreeing terms over a £5.5 million move.
Graham has seen hardly any play time since the arrival of Swans manager Michael Laudrup, with the Danish gaffer preferring to play Spanish sensation Michu in a lone striker position.
However Graham has been banging the goals in since being given a run in the side recently, and that has persuaded a number of clubs of the player’s worth.
Graham was thought to be happy to remain at the Liberty Stadium and fight for his place in the team, especially in light of his club’s successful run to the Capital One Cup final where they will face-off against League Two’s Bradford.
But it now appears the offer of regular first team football is too much to turn down as the player looks set to make his move to the Stadium of Light.
